OGS Library Patron and Volunteer Usage Consent During COVID-19 Pandemic

By visiting the Oregon Genealogical Society, Inc. & Research Library I knowingly and willing consent to the following guidelines:

I understand that COVID-19 has a long incubation period during which time carriers of the virus may not show symptoms and still be highly contagious.  It is impossible to determine who has it and who does not, given the current limits in virus screening.

I confirm that I do not currently have nor have I had any of the following COVID-19 symptoms in the last 14 days:
  • Fever
  • Shortness of breath
  • Loss of sense of taste or smell
  • Dry cough
  • Runny nose
  • Sore throat
To prevent the spread of contagious viruses and to help protect each other, I agree to follow the guidelines set in place by the State of Oregon and/or the Oregon Genealogical Society, Inc. & Research Library.
  • I will wear a face mask at all times while in the library.
  • I will sanitize my hands when entering the library.
  • I will fill out the sign in sheet so I may be contacted if a problem arises.
  • I will wash my hands with soap and water for a minimum of 20 seconds before handling printed materials.
I understand that travel significantly increases my risk of contacting and transmitting the COVID-19 virus.  I verify that I have not traveled by air, bus, or train in the past 14 days.
